What the pattern is responsible for

Shoulder adduction and internal rotation, driven mainly by pectoralis major, with a real triceps and anterior-delt contribution. Incline angle shifts emphasis toward the clavicular head, flat/decline toward the sternal head — see the chest anatomy lesson.

Tier 1 — Equivalent

Dumbbell press ↔ Machine press (Hammer Strength-style, converging path), at the matched incline angle. Both allow a genuine stretch at the bottom that a fixed barbell path cannot reach; the machine trades some stabiliser demand for the ability to safely reach true failure.

Barbell press ↔ Dumbbell press, at the matched angle. The barbell allows more total load; the dumbbell allows a deeper stretch. Neither is strictly better for hypertrophy — they load the same head through a similar range with different depth ceilings.

Tier 2 — Close, with a stated cost

Weighted dip, for a flat or slightly-declined press. Genuinely trains the sternal head, triceps and anterior delt hard, at real depth. The cost: the resistance profile is bodyweight-plus-load rather than a smooth barbell curve, and shoulder mobility requirements are higher — deep dips are a common source of anterior shoulder discomfort in lifters with limited external rotation.

Cable press (either angle). Constant tension through the range, unlike free weights where tension drops near lockout. A real substitute for the pressing stimulus; the cost is less total loadable weight than a barbell.

Tier 3 — Last resort

Push-up variations, weighted. Fine at low training ages or for maintaining the pattern with minimal equipment. The load ceiling caps out well below what a barbell or machine allows, so this is not a long-term substitute once you are progressing meaningfully.

If it's your shoulder, specifically

Depth is usually the trigger. Reduce range at the bottom first — stop the descent a few centimetres higher — before abandoning pressing entirely. If anterior shoulder pain persists at a reduced range, switch to a neutral-grip (palms facing each other) dumbbell or machine press, which removes much of the internal-rotation demand that irritates the joint.
