Overhead extension, pushdown, skull crusher — and why these are not interchangeable.

What the pattern is responsible for

Elbow extension. See the triceps anatomy lesson for the full mechanism — the short version is that overhead position and arm-at-side position train genuinely different heads well, and this is the one substitution category in this whole guide where the "obvious" swap is often the wrong one.

Tier 1 — Equivalent

Overhead cable extension ↔ Overhead dumbbell extension ↔ Overhead barbell extension. All three stretch the long head at the shoulder in addition to the elbow-driven stretch. Genuinely interchangeable with each other.

Pushdown (rope) ↔ Pushdown (straight bar) ↔ Pushdown (angled bar). Interchangeable with each other for lateral and medial head training — grip attachment changes wrist comfort more than muscle targeting.

What is not a tier-1 substitution, despite looking like one

Pushdown substituted for overhead extension, or vice versa. These are not interchangeable, and treating them as such is the single most common triceps-programming error. A pushdown trains the lateral and medial heads well and the long head — the largest of the three — poorly, because the arm-at-side position never stretches it. If your program calls for an overhead extension and you swap in a pushdown "because it's still triceps," you have quietly dropped direct long-head training rather than substituted for it.

Tier 2 — Close, with a stated cost

Skull crusher, bench angle depending. Roughly overhead-equivalent when performed on an incline bench with the upper arm angled back; closer to pushdown-equivalent when performed flat. The bench angle is doing the real work here, not the exercise name.

Close-grip bench press, dip. Both give a real triceps stimulus as part of a compound movement, with the added complexity that shoulder position varies through the rep, so neither cleanly isolates one head the way a dedicated extension does. Useful additions, not substitutes for the isolation work.

If it's your elbow, specifically

Reduce range at the bottom of the movement first — the deepest part of an extension is usually where elbow discomfort concentrates. If overhead extensions specifically bother the elbow or shoulder, a skull crusher on a moderate incline is the closest available substitute that still meaningfully stretches the long head.
